Rule 5
Application for trade union association:

(1) In accordance with sub-section (3) of section 4 of the Act, the official appointed by the working committee to register the trade union association shall submit an application to the registrar in the format as per Schedule-3, attaching the registration fee of five hundred rupees. (2) The following details must also be clearly disclosed with the application as per sub-rule (1):
(a) Name and address of trade union association.
(b) Date of Formation of Trade Union Association.
(c) Three copies of the statute as per section 10 of the Act.
(d) A copy of the agreement between the trade unions at the establishment level for forming the trade union association.
(e) Name of trade unions at the establishment level forming the trade union association.
(f) Names, addresses and nature of work of such workers if the workers working in the same nature of industrial business outside the establishment have formed a trade union association by mutual agreement.
(G) Name, Surname and Address of Officers.
(h) A copy of the decision made by the working committee regarding the application to the registrar for registration.
(i) A letter from the working committee regarding authorizing an official to submit an application to the registrar for registration.
(j) A document acknowledging that the trade unions at the establishment level who are members of the trade union association to be registered have not accepted the membership of another trade union association.
